DaVita has launched a value-based care agreement with Humana that, since July 1, is intended to serve more than 10,000 Medicare Advantage members with chronic kidney disease (CKD) stages 3B to 5. DaVita Integrated Kidney Care provides coordinated care focused on cardiovascular, renal and metabolic needs, emphasizing early interventions to stabilize kidney function and delay disease progression. The model leverages DaVita’s network of about 3,000 partner nephrologists and includes care coordination, education on home dialysis and transplantation, and support for barriers such as nutrition, transportation and mental health.
